Step 1. Print off all forms: Activity Dates Schedule, Fundraiser Packet, and Physical Form.

Step 2. Click the "Registration Open" link below. You will be routed to our registration website.  Follow the registration instructions.

Step 3. Print off the registration confirmation.

PLEASE NOTE:  Upon equipment issue, you will need Fundraising Money, Fundraiser Forms, & Physical

*If you have outstanding football equipment, cheer equipment, or owe money to CYFA, your child will not be allowed to participate until your account is settled.     Turn in Copy of Birth Certificate to coach.
