Application Deadline for Cheer and Football is July 1!

We have several opportunities for financial assistance.

*All players/Cheerleaders receiving Financial Assistance may be required to participate in the $75 Fundraiser and may be asked to volunteer as a field marshal, concession worker, set up/clean up helper*


TO APPLY FOR A SCHOLARSHIP or REDUCED REGISTRATION please follow these instructions:

  1. Register the participant for Football or Cheer

  2. Print and fill out the application form below

  3. Mail application and a copy of the front page of last year's Federal Income Tax Return (1040, 1040A, 1040EZ) to be considered 

  4. Sell one or more business ads or shoutouts for the CYFA Program

  5. All applicants will be notified of assistance awards by July 15

MAIL APPLICATION AND TAX RETURN TO: PO BOX  2712, Claremore 74018 - Must be post marked by July 1



**** PLEASE NOTE****

Returning players will not be able to check out equipment without a notice of award of assistance or payment in full of the registration fees.  If an application for assistance is still pending, registration fees will have to be paid in order to check out equipment.
